    1. Form 1: Student Visa Information
      Provide a copy of your F-1/J-1 visa to OGS, or confirm that you do not need a F-1/J-1 visa to enter the US.
    2. Form 2: US Immigration Requirements Training Confirmation
      Confirm completion of the mandatory self-paced online module.
