Requirements

The requirements below are mandatory. You can face serious consequences if you don’t complete the requirements in time (including being withdrawn from classes).

Submit Your Health and Immunization Forms

Deadline: May 15 (for students starting in Summer 2026)
Deadline: July 15 (for students starting in Fall 2026)

Proof of Immunizations

Upload documentation and enter dates for each required vaccine in the SHC Portal.

Important Document Submission Guidelines:
  • Do not submit password-protected files — they will not be accepted.
  • Do not send links to online documents — we do not open links.
  • Any documents in languages other than English must be accompanied by a certified translation (at your expense). Self-translations are not accepted.

You will receive an email to your NYU email address within 7-10 business days of completing these steps to notify you of whether or not you have met the immunization requirements. 
*Uploading your records more than once will delay processing time.

All students born on or after January 1, 1957 and taking six credits or more must provide documentation of proof of immunity to measles, mumps, and rubella (MMR).

You must provide ONE of the following to comply with this requirement:

  • Official record that you have received two (2) immunizations against measles, with the first one administered on or after 12 months old and the second one a minimum of 28 days later, AND at least one (1) immunization against mumps and one (1) immunization against rubella, both administered on or after 12 months old.
    – or –
  • A Student Immunization History Form (PDF 286 KB) completed and signed by a healthcare provider (MD, DO, NP, RN).
    – or –
  • Serological evidence (blood test) proving immunity for measles, mumps, and rubella.

All Students Under 21 Years of Age

All students under age 21 taking six credits or more must provide proof of vaccination against Meningococcal Meningitis (serogroups A,C,W,Y) on or after your 16th birthday and within the last five years.

All Students 21 Years of Age or Older

All students 21 years of age or older taking six or more credits must complete the meningitis decision form to remain in compliance. This form can be accessed via the SHC portal and is required regardless of whether you plan to receive the meningitis vaccine or opt out of it.

All students are required to provide information about their prior measles, mumps, and rubella (MMR) and meningitis vaccinations. Failing to meet this requirement can result in de-enrollement from classes. As of May 11, 2023, COVID-19 vaccination is no longer required, but students are encouraged to stay up to date with their COVID-19 vaccination.

Treatment of a Minor Form

If you are under 18 years of age, you must complete and upload the Treatment of a Minor Form so you can access services at the Student Health Center.

"Think About It"

Deadline: August 30 (For students starting in Fall 2026)

In an effort to foster a culture of sexual and relationship respect, all incoming degree seeking students are required to complete this online education course on sexual misconduct prevention and response at NYU in the first semester of their program.

All undergraduates students will be invited to take a second sexual respect training at the start of their third year at NYU. This second training acknowledges that students are in a different place in their journey in their third year, as compared to their first, and are viewing this important subject with a new lens given the experiences they will collect while living and growing as an NYU student.

"AlcoholEdu for College: Parts 1 and 2"

Deadline: August 30 (Part 1) and September 30 (Part 2) (For students starting in Fall 2026)

All incoming undergraduate students are required to take this two-part course—even those who have taken it at another institution, are over 21, or do not drink alcohol for personal, health, religious or other reasons—which provides basic knowledge of the effects of alcohol, potential problems, and ways to reduce risk, as well as resources to help you and others. If you have questions about this requirement, please contact alcoholmodule.support@nyu.edu.

  • You will receive an email invitation to complete the course a few weeks before the deadlines for Part 1 and Part 2.
  • It is important to complete Part 1 by the deadline so you can also complete Part 2 on time. Part 2 is due 30 days after you complete Part 1.

Health Insurance for Students Studying Away

NYU requires that students studying away as part of their educational program be covered under the NYU-sponsored Study Away health insurance plan administered by Worldwide Insurance Services. The school- sponsored GeoBlue program utilizes the Blue Cross Blue Shield provider network in the United States and provides easy access to care. Students enrolled in these GeoBlue plans are still required to maintain other health insurance coverage in their home countries.

Obtain prescription refills prior to arriving at NYU. Visit nyu.edu/health/global and select your site to learn more about traveling abroad with prescriptions and medication.

The Moses Center for Student Accessibility provides services and programs for students with disabilities. Please refer to the Moses website for information about arranging for accommodations before or after you start at NYU. You can request accommodations at any time, however, doing so early will help to ensure you have access to services the moment you start at NYU.

Tuition insurance is available and is recommended to protect against unanticipated financial consequences if a student must request a leave of absence because of physical and/or psychological illness.
