Online Degree Students
Welcome to NYU! In order to create a safer, healthier student body, there are mandatory health and immunization requirements you are required to complete to be compliant with New York State Public Health Law 2165 and 2167 and NYU policy.
The requirements on this page apply to a limited number of students in online-only degree programs. If you need to access any NYU buildings or participate in any in-person University-sponsored activities, such as internships, practicums, field placements, immersion experiences, field trips, and events, at any point during your program (including immersion programs), please complete the requirements for New York Students instead.

Requirements
The requirements below are mandatory. You can face serious consequences if you don’t complete the requirements in time (including being withdrawn from classes).
Submit Your Health and Immunization Forms
Deadline: July 15 (for students starting in Fall 2023)
Proof of Immunizations
All students are required to provide information about their prior measles, mumps, and rubella (MMR) and meningitis vaccinations. Failing to meet this requirement can result in de-enrollement from classes. As of May 11, 2023, COVID-19 vaccination is no longer required, but students are encouraged to stay up to date with their COVID-19 vaccination.
Student Health History Form
This form will help us better understand your health needs and assist us in providing you with the best care and treatment.
- Log in to the SHC Student Portal
- Click "Medical Clearances"
- Locate the row for "Health History Form"
- Click "Update" to begin
Treatment of a Minor Form
If you are under 18 years of age, you must complete and upload the Treatment of a Minor Form so you can access services at the Student Health Center.
- Download and complete the Treatment of a Minor Form (PDF 86.3 KB)
- Log in to the SHC Student Portal
- Click "Medical Clearances"
- Locate the row for "Consent to Treat a Minor"
- Click "Update"
- Upload a scanned copy or photo of the completed form
Complete the Online Trainings
Note: You must activate your Net ID to access the online trainings.

Deadline: August 30 (For students starting in Fall 2023)
In an effort to foster a culture of sexual and relationship respect, all incoming degree seeking students are required to complete this online education course on sexual misconduct prevention and response at NYU in the first semester of their program.
All undergraduates students will be invited to take a second sexual respect training at the start of their third year at NYU. This second training acknowledges that students are in a different place in their journey in their third year, as compared to their first, and are viewing this important subject with a new lens given the experiences they will collect while living and growing as an NYU student.

"AlcoholEdu for College: Parts 1 and 2"
Deadline: August 30 (Part 1) and September 30 (Part 2) (For students starting in Fall 2023)
All incoming undergraduate students are required to take this two-part course—even those who have taken it at another institution, are over 21, or do not drink alcohol for personal, health, religious or other reasons—which provides basic knowledge of the effects of alcohol, potential problems, and ways to reduce risk, as well as resources to help you and others. If you have questions about this requirement, please contact alcoholmodule.support@nyu.edu.
- You will receive an email invitation to complete the course a few weeks before the deadlines for Part 1 and Part 2.
- It is important to complete Part 1 by the deadline so you can also complete Part 2 on time. Part 2 is due 30 days after you complete Part 1.
Waive or Enroll in NYU-sponsored Student Health Insurance
Deadline: September 30 (For students starting in Fall 2023)
Most students are automatically enrolled in and charged a premium for an NYU-sponsored Student Health Insurance plan. Beginning June 27, students who maintain alternate health insurance coverage that meets the University’s minimum health insurance criteria may waive the NYU-sponsored student health insurance plan entirely and have the charge removed from their bill.
