Organizations can apply to receive funding from the Community Fund either by submitting the online application, or by printing and mailing the paper application. The deadline for all applications is Friday, April 30, 2021.

Instructions

Please have the following supplemental documents prepared before beginning the online application:

  • Budget for project for which grant is requested (list specific expenditures)
  • Fiscal data:
    • Evidence of tax-exempt status (must attach proof of 501(c)3 status)
    • Recent financial statements
    • List of board or committee members
    • Organizational chart of staff

You will be asked to upload the supplemental documents (preferably in a zip file) at the end of the form.
