UCAIHS ELECTRONIC SUBMISSION PROCESS


The UCAIHS Office no longer accepts paper copies of protocols. All submissions must be sent electronically to apply.humansubjects@nyu.edu.  Paper applications will be returned without review.
By submitting your application electronically, UCAIHS can ensure:

  1. that all submissions are time/date stamped;
  2. all submissions are uploaded into our developing electronic research administration module; and
  3. all research protocols are electronically tracked.

Frequently Asked Questions (FAQs)

Q: What about signatures?
A: Investigators should submit copies of their protocols in MS Word (for PC) and attach a scanned copy of the signature page in PDF format.
Q: What if some of my documents are only available in paper?
A: Any documents that cannot be transmitted in MS Word (such as signatures) should be scanned into PDF versions and attached. These may include:

  1. The signed signature page of the application;
  2. Research measures that are not electronic;
  3. Copies of dissertations and theses and/or their approvals
  4. Copies of grant proposals

Q: I don’t have a scanner – how am I supposed to scan my paper documents?
A: Please check with your school or department as most have scanners available. If not, please make an appointment with the UCAIHS office so that you can have your documents scanned. Note that an appointment will be necessary to access the scanner.
Q: What about my consent forms?
A: At this time, all consents (including fliers, parental permission forms, advertisements, etc.,) are to be sent only in MS Word format. This allows the UCAIHS Office to electronically stamp them as well as make minor corrections (typos, old contact information, etc.). Once the consent format has been reviewed and approved, the UCAIHS Office will stamp the forms, convert them to PDF and return them for your use.
Q: What about letterhead?
A: Your School or Department should be able to provide you with the appropriate electronic letterhead for your consent forms.
