Our Care Management Specialist offers comprehensive care and support for health and wellness related issues. A social worker is available to assist with a variety of emotional, medical and financial issues including:
- domestic violence
- unplanned pregnancy
- chronic health problems
- alternative or low cost medical care or medication
- short-term crisis intervention
- referrals to long-term counseling
