Health Information Management Services (HIMS) is responsible for the maintenance, disclosure and privacy of health records on patients seen at NYU Student Health Center (SHC). Health records are maintained in compliance with both federal and New York State laws.
If you plan to utilize the SHC and have an on-going medical condition (e.g. diabetes, epilepsy, heart disease, allergies, etc.), please ask your current healthcare provider to forward a summary of your medical status and all pertinent information to HIMS. Please ensure that this information is identified with your name, date of birth and student I.D. number. The summary should document current medications, allergies, immunization history and current diagnoses and treatments. This information will be included in your confidential health record and will be available to SHC providers.
Please Note: SHC is not permitted to provide you with a copy of such medical documentation obtained from another healthcare provider. Such a practice constitutes "re-disclosure of information." Therefore, you are encouraged to obtain a second copy of this information for your personal files as you may need it for your future healthcare concerns. Also, prior to submitting proof of MMR immunizations and meningococcal meningitis vaccination (see Immunology Services for requirements), you are advised to photocopy these records for your personal files as you will need to produce them in the future for various reasons (e.g. transferring to another school, traveling, etc).
Authorization for Treatment of a Minor:
For those students who are under 18 years of age, a parental or legal guardian authorization for medical treatment form must be on file in our office in order for you to receive prompt care and treatment, should the need arise. If student is under the age of 18, ensure completion of the Authorization for Treatment of a Minor form and return it to Health Information Management Services as soon as possible, if you have not already done so.
Advance Directives:
As a patient, you have the right to identify and/or designate a personal representative to act on your behalf with respect to health care decisions and uses and disclosures of protected health information (PHI) by the New York University Student Health Center (SHC) should you be incapacitated and unable to make decisions regarding your care and treatment. Retention of Health Records:
New York State law requires SHC to maintain your health records for a minimum of 6 years after the last date of service. Records of minors must be kept until the age of majority (18 years of age in New York State) plus the Statutes of Limitations. You may request a copy of your health records during these retention periods.
To Request a Copy of Your Health Record:
If you wish to obtain a copy of your health record, you must submit a written request to the HIMS Office (see address above). You may use the request form Authorization for Release of Health Information (PDF file) for a printable copy. All information must be completed in order for your request to be processed.
HIV/AIDS:
If your health records contain information relating to HIV or AIDS, New York State Department of Health requires a special authorization form - Authorization for Release of Confidential HIV Related Information (PDF file). You may visit our department and complete the necessary forms on-site or you may fax or mail your completed request form to our department.
We fax medical records to healthcare providers only in the event of a medical emergency.
PLEASE NOTE: HIMS will provide you with copies of radiology reports (written results of your x-rays). However, if you need copies of your radiology films, you must contact Radiology Services directly at 212-443-1072.
FEES:
Health records will be sent to another healthcare provider free of charge as a professional courtesy. However, records not going directly to another healthcare provider, are copied at the rate of 75 cents per page. Health records will be released upon payment of all fees. If you have any questions regarding the release of your health records, please contact the HIMS Correspondence Unit at 212-443-1272.
Changes to Our Privacy Practices:
We may change our privacy practices from time to time. If we do, we will revise the Notice of Privacy Practices, which will apply to all health information.
If you have not already received a paper copy of the Notice of Privacy Practices, you have the right to obtain one. You may also obtain a copy of all revised Notices by contacting the Student Health Center. We will also post all revised Notices at the Student Health Center.
