Immunization FAQs

All students born after December 31, 1956 who are registering for 6 or more credits must comply with the following New York State Public Health Laws.

NEW YORK STATE PUBLIC HEALTH LAW 2165 requires all college students registering for 6 or more credits in a degree-granting program to provide immunization documentation for Measles (Rubeola), Mumps, and Rubella (German Measles) prior to registration. Students born before January 1, 1957 are exempt. Failure to comply with the state immunization law will result in denial of registration privileges.

Measles, Mumps & Rubella FAQ Sheet (PDF file)

New students: Complete the Measles, Mumps and Rubella (MMR) section of your Student Health History form (you will receive this form by mail).

Continuing Students: Complete and submit a Student Immunization Record Form (PDF).

  • MMR (Measles, Mumps and Rubella) if given instead of individual immunization. The 1st dose must be on or after first birthday, AND on or after Jan 1, 1972. The 2nd dose must be at least 15 months after birth or later, AND at least 28 days after 1st dose.

- OR -

  • Measles: Two Doses
    1st Dose on or after January 1, 1968 AND on or after first birthday OR medical proof of history of the disease OR laboratory proof of immunity (must submit a copy of lab report)
    2nd Dose on or after January 1, 1968 AND on or after 15 months of age AND at least 28 days after first dose
  • Mumps: One Dose
    One dose on or after first birthday OR medical proof of history of the disease OR laboratory proof of immunity (must submit a copy of lab report)
  • Rubella: One Dose
    One dose on or after first birthday OR laboratory proof of immunity (must submit a copy of lab report)

NEW YORK STATE PUBLIC HEALTH LAW 2167 requires institutions, including colleges and universities, to distribute information about Meningococcal disease and vaccination to all students registered for 6 or more credits, whether they live on or off campus. All students registered for 6 or more credits are required to decide whether or not to be immunized for Meningitis and to provide formal confirmation of their decision.

New students: Must complete the Meningitis Response section of your Student Health History form (you will receive this form by mail).

Continuing Students: Must complete and submit a Meningitis Response form (PDF file).

Complete a response form with one of the following choices:

  • Had the Meningococcal Meningitis immunization (Menomune™) within the past 10 years - indicate the date
  • Have read or have had explained to me the information regarding meningococcal meningitis disease and will obtain immunization within 30 days
  • Have read or have had explained to me the information regarding meningococcal meningitis disease and will not obtain immunization
