
New UNDERGRADUATE Students - All new students must complete the Student Health History form*. The Student Health History form is a paper form that is mailed to all new undergraduate students (it is not available online for new undergraduates). It should be returned before the August 15, 2009 deadline in the provided postage paid return envelope.
Steps for completing the Student Health History form:
- Do not separate form.
- Submit original form ONLY. We cannot accept a fax or photocopy.
- Do not staple.
- Use pencil or pen (black or blue ink) only.
- Fill in all circles and boxes completely (including name and N#). Do not draw a single line through response.
- Use Month/Day/Year format for all dates (00/00/0000). If exact day is not known, please use the first day of the month (i.e. 05/01/2007).
- Use Wite-Out® to correct any errors. Do not cross out errors with an X.
- Return completed form in the supplied postage-paid business reply envelope (this envelope is included in the mailing all new students will receive).
PLEASE NOTE: We will be unable to process forms with incorrect markings or missing information.
* Students under the age of 18 must also submit the Authorization for Treatment of a Minor form.

New GRADUATE Students - All new students must complete the Student Health History form. The Student Health History form is now electronic for all new graduate students. It should be completed before the August 15, 2009 deadline.
Steps for completing the Student Health History form (for graduate students) online:
- Log into NYU Home
- Click the NYU Life tab
- Under the Health & Wellness section, click On-line Services
- Log into OpenCommunicator using your NYU Net ID and password
- Click on Forms and follow the directions

- Where do I get the Student Health History form?
The Student Health Center mails a packet of information regarding health requirements to all new incoming students.
The Student Health History form and Student Immunization History form (see Immunization FAQ's for details on immunization requirements) are included in this packet for new undergraduate students.
New graduate students will complete the Student Health History form online and receive an Student Immunization History form (see Immunization FAQ's for details on immunization requirements) in the packet.
- What if I never received the Student Health History form?
If you are a new undergraduate student and you did not receive the form in the mail, please send an email to health.requirements@nyu.edu and provide us with your complete address. We will mail a form to you.
- Why do you need the information requested on the Student Health History form?
This information will help us better understand your health needs and assist us in providing you with the best care and treatment when, and if, you need us.
- Do I need to have a physical examination?
You do not need a physical examination, however you are required to fulfill a number of health requirements:
If you are a new undergraduate student you are required to:
- Complete the self-reported medical history on the Student Health History form and submit the form to the Student Health Center
- Complete the AlcoholEdu® course
If you are a new graduate student you are required to:
- Complete the self-reported medical history online.
- When is the Student Health History form due?
We ask that you return/complete the Student Health History form and Student Immunization History form no later than August 15, 2009 if you will be a new student in Fall 2009. The deadline is critical in allowing us time to process the information in preparation for new students' arrival to NYU.
- Where do I send the Student Health History form?
New undergraduate students* should return the form and any required health history documents in the provided envelope to the Student Health Center - or they can be dropped off at:
