Dependents

Eligibility

Eligible dependents are:

• the covered student's spouse or same sex domestic partner; or
• the covered student's unmarried child under the age of 19 years (or 23 years if a full-time student). The child must be fully supported by the covered student.

Enrollment

To enroll an eligible dependent, complete and submit the online Dependent Enrollment application. The application, together with full payment, submitted directly to Chickering Benefits Planning Insurance Agency Inc. Please review deadlines.

Dependents cannot enroll using the online waiver process. Dependents must enroll at the same time as the covered student unless there is a qualifying life event. Dependents are eligible for coverage under the Basic and Comprehensive Plans, but must enroll in the same plan as the covered student.

Payment

Dependents enrolling in an NYU plan before the Fall term deadline may choose an installment payment option. For dependents choosing this option:

• The policy is annual and the Spring term payment is required in order for coverage to be effective and continuous through August 20, 2010.
• You will receive a 30-day and 10-day notice before your Fall coverage ends with a request for payment for the Spring term coverage.
• You must send the appropriate payment for the Spring term by February 10, 2010. If no payment is received by February 10th, a letter will be sent informing you that your dependent coverage has been terminated.

Accessing Benefits

Dependents do not have access to services at the NYU Student Health Center at 726 Broadway. SHC services are exclusively for NYU students.

Dependents are encouraged to seek care from providers who participate in the Aetna network of healthcare providers to ensure maximum benefits and minimal out-of-pocket expenses. Dependents may, however, use any provider. To find a MultiPlan Network Provider go to www.MultiPlan.com. Select "Search for a Doctor or Facility" and choose the green MultiPlan logo Referrals for medical treatment are not required for dependents

Routine annual physicals for spouses and partners are not covered, except for well-woman care. Well baby visits up to the age of three, and immunizations for children up to the age of 12, are covered according to plan benefits.
