New York University and VillageCare invite you to attend Take Charge of Your Health Today!, 2nd Annual Community Health Forum, on Tuesday, May 14, 2013.

Members of the public - consumers as well as caregivers - will receive information and direction not only for some of our most pressing health care questions, but also for the legal and financial issues we all will face.

The evening will include:

5:00 PM Health Expo featuring local businesses and service providers

6:00 PM Panel Discussion and Audience Q&A

The panel of distinguished speakers will address topics including:

  • How to make the proper financial and legal arrangements for yourself and those in your care
  • How to manage long-term health care needs and be your own best health advocate
  • Accessing community resources to help manage chronic illness
  • How best to handle and get counsel on your health issues
  • Where to get help in setting and maintaining realistic goals for fitness and health
  • How to improve communication with your health provider and overcome barriers

The discussion will be moderated by Dr. Max Gomez, Emmy-winning CBS TV medical reporter. The panelists include:

  • Tara A. Cortes, PhD, RN, FAAN Executive Director; NYU College of Nursing, Hartford Institute for Geriatric Nursing
