Part Two: Grounds for Request
	NEW INFORMATION: New information is now available that was unavailable at the time of your initial hearing and this information would have made a difference in the outcome.
	PROCEDURAL ERROR: There was a procedural error in the judicial process that had a significant impact on the outcome. Please read the judicial procedures as published in the NYU Housing and Residence Life Handbook and identify the specific error that occurred.
	SEVERITY OF SANCTIONS: The sanctions that were issued are too severe in light of the violation and/or your prior disciplinary history.

Upon submitting this form, any one of the following actions may be taken. A review will be granted and an individual meeting to discuss the matter will be scheduled with the appropriate administrator. A review will be granted and sanctions reduced as deemed appropriate. The review will be denied and all findings and sanctions of the initial hearing will be upheld.
