Only applications for Cooperative Agreements and other complex mechanisms, as well as for a Change of Grantee Institution, will routinely use a paper application (PHS 398, revised 06/2009): http://grants.nih.gov/grants/funding/phs398/phs398.html
For deadlines falling on January 25, 2010, and thereafter, both form applications have been shortened and restructured to reflect agency-wide changes to peer review processes and evaluation review criteria.
