Name Change Procedure

All university correspondence, including mailings, certificates and diplomas are addressed to the name on record in Albert.

To change your name, you must submit a Change of Name form to the Office of the Registrar. Obtain the form at the Student Services Center or download it from the appropriate link below.

Submit the completed form to the Office of the University Registrar at the Student Services Center or mail it to:

Diploma Name Adjustment

If you wish to add an accent or special marking to your diploma name and you are not processing a name change, fill out a form at the Student Services Center indicating your full name with the appropriate accents or markings. You may also mail or fax a signed statement including clear and full spelling of your diploma name with the appropriate accents or markings to:

Change of Name after Graduation

Requests from NYU graduates for a change of name after graduation are evaluated on an individual case basis. Requests for a change of name due to marriage, divorce, naturalization or a court-ordered change of name will be considered. The request should be made to:

In your request, please state the basis for the name change. A photocopy of the relevant legal documents should be enclosed. If the request is approved, the original diploma must be returned together with a check or money order in the amount of $60.00 payable to New York University. A diploma in the new name will be ordered, and the name will be changed on the academic record. Delivery is normally made three to four weeks after the order is placed.
