Supervisors
Human Resource Representatives
Report immediately all work related incidents, injuries and illnesses by using the NYU Insurance Department web site. You can click the button below to go directly there.
Print a copy of the incident report (by following instructions at the web site) and forward it to the employees HR Representative. Continue to report all lost time information to the employees HR Representative. If you are unable to access the web site, please contact the employees HR Representative in your School or Division, to report work related incidents, injuries and illnesses.
Call Suzanne Oommen, NYU Insurance Department (extension 998-2731), to report any change in the status of an employee's workers' compensation case, including additional lost time from work.
Please familiarize yourself with procedures which are required for employees and supervisors (see above).
If you become aware that a work related injury has occurred, confirm with the employee's supervisor that he/she has completed and submitted the NYU Work Related Incident / Injury / Illness Form on the NYU Insurance Department web site.
If a supervisor is not available, or is not able to access the web site, the employee must contact their Human Resources Representative, who will need to access the web site and complete the Work Related Incident/Injury Report for that supervisor, and forward it to the Insurance Department.
The Insurance Department will send confirmation of the work-related illness / injury report via e-mail or interoffice mail to the Supervisor, Human Resources Representative, Benefit Specialist in Human Resources, Environmental Services, and University Health Center.
