Workers' compensation is insurance that provides cash benefits and medical care for workers who are injured or become ill as a direct result of their job. Employees are not responsible for the cost of medical treatment when such treatment is directly related to a work related injury or illness.

Your employer (NYU) pays for workers' compensation insurance. You are not required to contribute to the cost of workers' compensation insurance.

Effective September 1, 2000, Liberty Mutual Insurance Company became NYU's insurance carrier for all work related injuries sustained on or after that date. The address of Liberty Mutual is: P.O. Box 4836, Syracuse, New York 13221-4836.

Suzanne Oommen, Manager, Workers' Compensation, in the NYU Insurance Department, will coordinate all claims reporting to Liberty Mutual. Call Suzanne Oommen at extension 998-2731, if you require assistance with your workers' compensation claim.

Your Liberty Mutual adjuster will contact you within 24 hours of receipt of your claim. The adjuster will ask your status, discuss appropriate medical care, and if appropriate, start the process to commence workers' compensation benefits as authorized under the NYS Workers' Compensation law.

Remember to inform your workers' compensation authorized medical provider, that medical bills for treatment related to your work related condition, must be sent to Liberty Mutual at the above address.

You can obtain your Liberty Mutual claim ID number from Suzanne Oommen, or from your Liberty Mutual claims adjuster at (800) 422-0820.

You may elect to receive your workers' compensation medical care from the University Health Center at 726 Broadway (corner of Waverly Place), or from any NYS Workers' Compensation Board authorized physician. You can obtain the name of an authorized physician by calling the Workers' Compensation Board at (800) 877-1373.
