Instructions For Requesting Film / Video Insurance
This procedure has changed to streamline the information gathering process. The first form you must fill out is the PRODUCTION REGISTRATION FORM. Here you will provide all the basic information needed for your entire shoot. Once this form has been submitted, if any changes occur with your production, you’ll need to use the PRODUCTION UPDATE FORM to make these changes. Using this new process will make filling out all the other forms much simpler.
Read and complete the entire form that you selected. It’s very important that you completely and accurately provide all the information requested on the form. The NYU Insurance Office will not process any request for insurance, without all the information submitted in a complete and accurate manner. Any mistakes or missing information will invalidate the request. You will then have to resubmit your entire request.
All Cast & Crew (NYU Students) must register with NYU Traveler for any and all travel, both Domestic & International, as well as any overnight stay away from your primary residence. Failure to do so will delay the issuance of insurance certificates.
Provided that your submission is complete and accurate, the NYU Insurance Office will issue a CERTIFICATE OF INSURANCE for each request. CERTIFICATES OF INSURANCE will be processed and distributed within 5 BUSINESS DAYS of receipt of your submission.

NOTE: If you checked off any SPECIAL INSURANCE REQUESTS on your insurance request form, it may take up to TWO WEEKS to process your request. PLAN ACCORDINGLY!!
After you submit your insurance request, you will see an acknowledgement that your request has been received by the Insurance Department. Please do not call the Insurance Department asking when your certificate will be ready! These phone calls cause heavy delays in the insurance certificate issuing process!

Note:
Emailing the certificate will require you to have the latest version of Adobe Acrobat Reader on your computer, and access to a printer. The Adobe Acrobat Reader can be downloaded for free by clicking the box below.

Step #3 - Delivery
There are two options for the delivery of your certificate. You can have it emailed directly to you, or you may pick up your certificate at the Insurance Office, located at 838 Broadway, 4th Floor, between the hours of 9:00 am to 4:30 pm, Monday through Friday (excluding holidays). Please make the appropriate selection on the submitted form.
It's your responsibility to pick up, or print out, the certificates, and deliver them to the appropriate Owner/Occupant, Mayor’s Office, or Rental House for your project.
THE INSURANCE OFFICE WILL NOT MAKE ANY OTHER ARRANGEMENTS FOR THE DELIVERY OF CERTIFICATES.
Note: Only 7 Certificates Will Be Processed Per Day, Per Production.

To avoid confusion, only the Director, or the Producer, should be sending correspondance to the Insurance Department. We will answer these questions in a timely fashion. Phone calls will not be accepted, and you will be referred to the above Contact Form.

For each Equipment Rental Vendor, Private Location Owner/Occupant, or non-New York City Public Entity:
A separate REQUEST FOR INSURANCE form must be completed and submitted for each entity.
For the Mayor’s Office of New York City:
One request form is used, and all of your locations are listed on that form.
