The Fales Library & Special Collections Public Services Forms

All patrons are asked to complete a Reader's Application form before consulting materials in the Fales Reading Room or receiving photocopies or scans remotely.

All patrons requesting to consult archive and manuscript materials in the Fales Reading Room are asked to complete the Manuscript Request form.

All requests for photocopies of Fales materials must be submitted in writing using the Photocopy Request form.

All requests for photographic duplication of Fales materials must be submitted in writing using the Photographic Request form.

All patrons wishing to quote from or publish materials held by the Fales Library are asked to complete the Notification of Intent to Quote or Publish form prior to publication and to submit it to the Fales Director at fales.library@nyu.edu.

All patrons wishing to publish photographic materials held by the Fales Library are asked to complete the Notification of Intent to Publish Photographs form prior to publication and to submit it to the Fales Director at fales.library@nyu.edu..
