Instructions For Special Insurance Requests
When you have selected any of the "Special Insurance Requests" boxes, please keep in mind that you must also fill out the corresponding questionnaire(s). These questionnaires will go to our Insurance Broker to determine if this coverage is possible to obtain, and if there will be an additional charge for this non-standard insurance coverage. The additional cost will be paid by you, directly to the Insurance Department, via check or money order. These questionnaires can be accessed by clicking the corresponding buttons below.
Because of the process that takes place between our Insurance Broker and the Insurance Carrier, you can expect up to a TWO WEEK delay in approving and obtaining this type of insurance.
PLEASE PLAN ACCORDINGLY!!
Example:
If you checked the "Firearms, Squibs, Etc." box and the "Bridges, Water Scenes, Etc." box, you will have to complete two different Special Insurance Request forms. First click on the "Firearms, Squibs, Etc." link and fill out the necessary information, and then go to the "Bridges, Water Scenes, Etc." link and fill out all of that information as well. Your original request for insurance will not be processed until all the Special Insurance Request Forms are filled out, and submitted to the Insurance Department.
FAILURE TO REPORT ALL SPECIAL INSURANCE REQUESTS ON YOUR SHOOT, CAN RESULT IN NO INSURANCE COVERAGE FOR YOUR PRODUCTION!!
