Instructions For Requesting Film / Video Insurance
Before you fill out the form below, upload your Production Book to Google Drive if you have not already done so. For a complete checklist of what is required, please refer to the following links.

Insurance & Risk Management Department
All curricular film and video activity must be registered with and approved by the Department of Risk Management if any of the conditions are met below:
1. You are shooting on private property not owned by NYU.
2. You require a shooting permit to shoot on public property.
3. You are renting equipment or props.
4. You are filming any potentially hazardous scenes or activities.
Please do not proceed with registration until after you have been approved by your teacher or your Production Supervisor.

Fill out the Insurance Registraion form below. It's very important that you enter location and vendor information exactly how they should be written on the certificate of insurance.

To avoid confusion, only the Director or the Producer should fill out the form.
IMPORTANT: Before you submit the form, make sure you check the box marked "Send me a copy of my responses". This is the only way you will be able to make changes or additions once the form is submitted.
Provided that your submission is complete and accurate, the NYU Insurance Office will issue a CERTIFICATE OF INSURANCE for each location and vendor indicated.
TURNAROUND: Certificates will generally be processed and distributed within 5 BUSINESS DAYS of receipt of your submission. If you checked off any SPECIAL INSURANCE REQUESTS on your insurance request form, it may take up to 10 BUSINESS DAYS (2 WEEKS) to process your request. Please plan accordingly.
DELIVERY: There is one option for the delivery of your certificate. It will be emailed directly to you, between the hours of 9:00 am to 4:30 pm, Monday through Friday (excluding holidays).
It's your responsibility to print out the certificates and deliver them to the appropriate Owner/Occupant, Mayor’s Office, or Rental House for your project.
THE INSURANCE OFFICE WILL NOT MAKE ANY OTHER ARRANGEMENTS FOR THE DELIVERY OF CERTIFICATES.
NOTE: Emailing the certificate will require you to have the latest version of Adobe Acrobat Reader on your computer, and access to a printer. The Adobe Acrobat Reader can be downloaded for free by clicking the box below.
Step #3 - NYU Traveler
All NYU students in your cast & crew must register with NYU Traveler for any and all travel, both domestic & international, as well as any overnight stay away from their primary residences. Insurance certificates will not be issued until we receive copies of all registration confirmation emails.
