COLLECTIVE BARGAINING AGREEMENT Between NEW YORK UNIVERSITY And LOCAL ONE SECURITY OFFICERS UNION

ARTICLE 28 - HEALTH, DENTAL, AND LIFE INSURANCE

A. Health Insurance

The employer shall continue to provide its present choices of health insurance coverage, or substantially equivalent coverage, on an individual basis and for families to full-time employees upon completion of their probationary period, at no cost to the employee for the duration of this Agreement. Employees hired after May 6, 1997 will only have a choice of HMO plans. Employees hired before May 6, 1997 may switch to the University's indemnity plan during the annual open period through 1998. After 1998, Employees may change to an HMO or a different HMO but may not switch to the indemnity plan.

B. Dental Insurance

The University will continue to provide its Met Life Dental Assistance Benefits Plan, or equivalent coverage, (including the provisions setting forth exclusions, limitations, deductibles, and service requirement) on an individual basis to full-time permanent employees at no cost to the employee for the duration of this agreement. Coverage for the family, including a domestic partner and children of the domestic partner, of a full-time permanent employee is permitted on a shared cost basis. The contribution for family coverage is $4.00 per month. The service requirement is three months. Effective January 1, 2002, the benefit schedule shall be improved to the same level as provided by the University to administrators. The University shall continue to provide the same coverage that it provides for administrators for the duration of this Agreement.

C. Group Life Insurance

1. Effective February 1, 2002, the Employer shall grant $20,000.00 (increased from $15,000.00) of the Employer's group life insurance coverage at the Employer's expense to all full-time employees covered by this Agreement upon the completion of the three months probationary period of employment.

2. Optional additional amounts will be available to full-time employees in $5,000 increments, up to a maximum of $30,000, on a cost-shared basis, with the employee paying two-thirds of the cost and the University paying one-third. Effective February 1, 2002, the maximum will be increased to $40,000.

3. New employees have 30 days after completion of the probationary period to enroll without proof of insurability.

4. Employees who choose not to elect voluntary coverage during the initial enrollment period available to them will have to show proof of insurability at a later date in order to participate.
