Pre-Employment Physical Process: Information and Forms

Prospective employees for code 107/117 positions must pass a pre-employment physical before they are to begin work.

FORMS:

• Pre-employment Physical Examination Form

Prospective employees must complete the history portion of this form PRIOR to seeing the healthcare provider at the second appointment.

• Waiver Form

Prospective employee must complete this form prior to the first appointment.

• Pre-Employment Physical Clearance Form

HR Officer will complete the designated fields in this form. UHC will complete "clearance" section and send to HR Officer indicating whether or not prospective employee is cleared to work.
