Child Care Subsidy Claim Form

The Child Care Subsidy Fund is a financial assistance program established by NYU and the United Staff Association, Local 3882 to help NYU office and technical staff with child care related expenses. Child Care Subsidy applications are mailed each Fall to all eligible employees and completed forms must be submitted before the end of each calendar year. Fund allocation decisions are made by a joint University-Union committee during the month of January and recipients are notified of the amount of their award by the end of February. Requests for reimbursements may be sent to the claims administrator, United HealthCare, beginning March 15 of the Fund's calendar year, but no later than December 31 of the Fund year.

Instructions

For employees in Codes 106/116 and 104/114 who applied for and received a Child Care Subsidy.

This form is used to file for reimbursement of eligible child care expenses that you have incurred during the Fund's calendar year.

The claims administrator, United HealthCare, must receive requests for reimbursement no later than December 31 of the Fund's calendar year.
