PASS Transmittal Form

This form is used for personnel transactions processed through PASS which require additional documents that cannot be submitted electronically.

It is critical that this form and the required documents be routed immediately to the office(s) listed on the form. Most of the documents must be received prior to your request being processed into HRIS.
