PAF-EP Employment-Primary

This is a personnel action form and it is used to set up an employee in Human Resources Information System (HRIS) for the first time. If the employee is returning to the University and is on HRIS, a Change Primary form (CP form) must be processed instead.

If the employee has additional appointments upon initial employment, an Employment Non-Primary (EP form) should be attached to the Employment Primary form (see EP form). Only appointments set up in HRIS are paid.

Instructions

The Human Resources Representative or designee must approve this form. The Employment Office, the Office of Academic Appointments or the Office of Career Services must also approve the form. Forms should be processed in accordance with the schedule of payroll deadlines, which are posted on the HR website. The Office of Academic Appointments has a separate deadline schedule. Once personnel action forms are completed and approved within your unit the form should be forwarded to one of these offices.
