PAF-EA (Employment Addendum) Faculty

This personnel action form is an addendum to the originally submitted EP form. It is used to gather previous employment information on tenure-track faculty. This form should be completed, signed and attached to the EP form for all new full-time tenture-track faculty hires.

Note that information submitted on the EA is not keyed into Human Resources Information System (HRIS)
