PAF-CN Change-Non Primary

This is a personnel action form and it is used to set up or process any changes to an employee's non-primary appointment(s) and to have those changes reflected in Human Resources Information System (HRIS). This form is only used to process non-primary appointments for an employee with a current primary appointment.

Instructions

The Human Resources Officer or designee must approve this form. The Human Resources Division, the Office of Academic Appointments or the Office of Career Services must also approve the form.

Forms should be processed in accordance with the schedule payroll deadlines, which are posted on the HR website. The Office of Academic Appointments has a separate deadline schedule. Once personnel action forms are completed and approved within your unit the form should be forwarded to one of these offices.
