Time Sheet-Dual Employment (Print Only)
This form is "print only" and cannot be filled in on-line.
This form is used to request approval and payment when a code 106 or 104 employee works additional hours in another department or school/division. It is also used to pay hours (straight or overtime) worked for this "second job" or when a unit wants to
charge hours for an employee's primary job to a different account. The Compensation office approves dual employment.
Instructions
This form should be completed by supervisor and approved by Human Resources Officer or designee, then submitted to the Office of Records and Information Services. Forms should be processed in accordance with the schedule of payroll
deadlines, which are posted on the HR website.
