Exit QuestionnaireThe Exit Questionnaire is to be completed by Administrative/Professional (code 100), Office/Clerical and Technical/Laboratory (code 106/104) and Service (code 107) employees who have voluntarily resigned from New York University. This questionnaire should not be completed if you are a faculty member, causal or temporary employee or if you are transferring within the University. Thank you for taking a few minutes to complete this questionnaire. As you leave the University, you have a unique perspective on the strengths and weaknesses of our organization.
