IT-2104 New York State/City/Yonkers Employee's Withholding Allowance

This form is for New York State, New York City or Yonkers residents who elect to claim additional allowances or request additional withholding amounts than noted on their W-4. This form must be completed, signed, and submitted to the Payroll Department for processing.
