W-5 Form - Tax Form for Earned Income Credit Advance Payment

This form is used by employees claiming "Earned Income Credit Advance Payment". This form should be completed and filed by employees who have determined that they are eligible to receive a part of their federal tarx credit early. To determine if you are eligible, complete the questions on page 2 of the form.
