Bonding Leave Application (Code 100)

This form is used for Code 100 employees who have completed two (2) years of full-time service (excluding any leaves) to request paid leave to allow for a period of bonding with their newborn child, a newly adopted child or the sponsor of a new foster care placement. To be eligible, an employee must:

  1. Be a member of the Administrative & Professional staff, Code 100
  2. Have completed at least 2 years of continuous full-time service (excluding leaves) prior to birth, adoption or placement in foster care.
  3. Have met all the conditions stated in the bonding policy

Instructions

The employee requesting paid bonding leave must complete this form and have it signed by his/her supervisor, HR Officer and the Employee Relations Office.
