Life Insurance - Notice of Conversion Privilege

For all employees and former employees whose NYU group life insurance has terminiated.

NYU group life insurance through TIAA is convertible to ordinary life insurance. You may convert all or any part of the amount of insurance for which you were insured, subject to the limits of the policy. You have 31 days from the date of termination of the insurance in which to convert your group life insurance to an individual policy. The individual policy will be issued without medical examination at a premium based upon the rate applicable to your age on the effective date of the new policy.

Instructions

When your NYU group life insurance terminates, you may convert all or any part of the amount of insurance for which you were insured to an individual policy. You have 31 days from the date of termination of the insurance in which to convert your group life insurance to an individual policy.
