Annual Increase/Reappointments/Upload Authorization
This form is used to notify the Compensation Office or the Office of Academic Appointments that annual increases or reappointments authorized by your unit have been uploaded for Central Administration review and approval.
Instructions
Forms for administrative appointments (object codes 100 and 110) are forward to Human Resources Division, Compensation Office at 7 E. 12th Street, 2nd fl.
Forms for academic appointment (object codes 102, 112, 103, 113, 101, 111, 130, 131, and 542) are forward to the Office of Academic Appointments at 7 E. 12th Street, 10th fl.
This form must be completed and submitted for your initial upload and all subsequent uploads.
