IN YOUR NETWORK

The health plans provided by NYU offer a number of laboratory facilities in locations that should be easy to reach from your work site, or from home.

If you need to find a lab that participates in your network, you should use your health plan's online provider referral directory.

Aetna** HMO: Log on to Aetna Navigator® and use DocFind, Aetna's online participating provider directory at www.aetna.com/docfind.

HIP Prime HMO: For the HIP Prime network log on to www.hipusa.com or call the toll-free Customer Service Line,1-800-HIP-TALK (1-800-447-8255).

Oxford Freedom HMO: Log on to www.oxhp.com/ to find a lab, or use LabCorp's Patient Service Center Locator at www.labcorp.com/psc. You may also call LabCorp's Customer Service Center at 1-800-631-5150.

United Healthcare: Log on to www.myuhc.com, use LabCorp's Patient Service Center Locator at www.labcorp.com/psc or call LabCorp's Customer Service Center at 1-800-631-5150.

AND REMEMBER: When visiting a laboratory facility, you should bring the following information:

  • A request form from your health care provider for lab tests
  • Your current insurance ID card
  • A photo ID (e.g., driver's license, employee identification badge)
  • Minors must be accompanied by their parent or legal guardian
