Addition to Need to Know by NICHE Library Counsels Patients with Dementia and Their Families on Transitions and Communications
 

NICHE (Nurses Improving Care for Healthsystem Elders) has added the Dementia Transition series to the Need to Know by NICHE library. The new patient and family education tool is designed and written to provide older adult patients with dementia and their families guidance to help them understand and cope with hospitalization and transitions. The four-part Dementia Transition series was developed by Maggie Murphy-White, MA, Alzheimer's Association - St. Louis Chapter, in partnership with NICHE. Family Caregiver Reports are included on the back of each document to communicate patient information to health care providers. The Need to Know Dementia Transition series is available to the public and can be found online at http://www.nicheprogram.org/need_to_know.

The Need to Know Dementia Transition series consists of  “Home to Hospital,” “Hospital to Home,” “Hospital to Long Term Care or Rehabilitation,” and “What to Communicate at the Hospital.” Transitioning to the hospital, long-term care facility, rehabilitation facility and back to the home can be a difficult and stressful time for patients with dementia. The change to an unfamiliar environment — and ultimately to home following treatment — is likely to increase confusion and worry. The series provides patients and their families with information and communication strategies to help them prepare for these transitions and thus relieve unnecessary anxiety and uncertainty.

About Alzheimer's Association - St. Louis Chapter
Founded in 1981, the St. Louis Chapter services 38 counties in eastern Missouri and western Illinois. The chapter is the developer of the “Dementia Friendly Hospitals: Care Not Crisis” program. The program provides education to hospital staff to improve care for people with dementia in the acute care setting. The goal is not the diagnosis of dementia, but rather awareness of symptoms of dementia that will affect care. The program is one of the many innovations NICHE hospitals adopt to help improve care of older adult patients.
