Complete the following Statement of Fees Request form and mail it to the Bursar's Office or fax to (212) 998-2817.

Email a Statement of Fees request to firstname.lastname@example.org. Please remember to include the following information in your email request:
- Full Name
- Student Identification Number
- School Registered In
- The semester or semester(s) you are requesting a statement for
- Daytime phone number where you can be reached
- Mailing address (if different from Student Information System/Albert address)
All Statement of Fees will be mailed to your permanent address as it appears in the University's Albert Student Information System via NYUHome. Please allow 2-3 days for processing.
- To update your address in ALBERT, log onto NYUHome with your Net ID and password.
- Click the ACADEMICS tab, then click the Student Center link.
- Scroll down to the Personal Information section to update your address.
